We require all patients with private health insurances to provide us with a card on file for payment purposes. You can read more about it here.
As a patient of the Diabetes & Hormone Center, you are responsible for your account with us. Please make sure we have your accurate address, phone, email and insurance information.Correct information is critical in filing insurance claims as well as in communicating with you. Please bring your current health insurance card with you at the time of service. Our practice accepts most insurance plans and a complete list is provided here.
You are responsible for deductible, coinsurance and any other out-of-pocket expenses - these are due on the day treatment is rendered. You can pay via cash or credit card prior to the physician visit. Please note that we do not accept personal checks and all pending charges are due prior to your next appointment.
If you are unable to keep your appointment, we ask that you give us at least 48 business-hours notice. Not doing so will result in a No-Show charge ($50 for follow-up appointments and $100 for new patient appointments).
In general prescriptions (and refills) are only provided at time of appointment. However, we understand special circumstances and as a courtesy we can authorize refill request only ONE time provided it is initiated electronically by your pharmacy. Patients are expected to make follow-up appointment for any refills that go beyond this one time extension.
