    Understanding Your Thyroid Lab Results

    If you have been treated for a thyroid condition, you have probably seen a lab report with several unfamiliar abbreviations on it. This article explains what the most common thyroid tests measure and why your results are interpreted alongside how you feel, not on their own.

    The tests you will see most often

    TSH (Thyroid Stimulating Hormone) is produced by the pituitary gland, not the thyroid. It rises when the body senses too little thyroid hormone and falls when it senses too much. Because of this inverse relationship, a high TSH generally suggests an underactive thyroid.

    Free T4 measures the amount of thyroid hormone available to your tissues. It is often checked together with TSH to distinguish between different causes of an abnormal result.

    Thyroid antibodies (TPO and thyroglobulin antibodies) help identify autoimmune thyroid disease such as Hashimoto's thyroiditis. These are usually checked once rather than tracked at every visit.

    Why "normal range" is not the whole story

    Reference ranges printed on a lab report describe the middle of a broad population. They are a starting point, not a treatment target. Your physician will interpret your results in the context of:

    • Your symptoms and how they have changed
    • Whether you are pregnant or planning to become pregnant
    • Other medications that affect thyroid hormone absorption
    • Your history of thyroid surgery, nodules, or radioactive iodine treatment

    Why results shift between visits

    Small changes between visits are common and usually do not mean your treatment has failed. Several everyday factors influence thyroid labs:

    1. Timing of your dose. Taking levothyroxine shortly before a blood draw can raise Free T4 without reflecting your usual level.
    2. Supplements. Calcium, iron, and biotin can interfere with absorption or with the assay itself. Biotin in particular can produce misleading results.
    3. Other illness. Acute illness temporarily changes thyroid lab values in ways that resolve on their own.

    For the most consistent results, take your thyroid medication at the same time each day, separate it from calcium and iron by at least four hours, and let our office know about any supplements you are taking before your blood draw.

    When to contact the office

    Call us if you experience a marked change in energy, weight, heart rate, or mood, or if you become pregnant while on thyroid medication. Dose adjustments are common and expected, especially in the first year of treatment.
